Outcomes Improvement and Cost Reduction
The purpose of this study was to determine if using home tele monitoring could improve outcomes in patients with diabetes as well as reduce costs to treat chronic diseases. A literature review of previously published studies was done to determine outcomes of this study. Utilizing portable technology in the patient centered home care model should provide providers with the ability to monitor their patients with chronic health conditions and improve treatment interventions. Home telehealth should also be utilized to reach patients who otherwise have difficulty accessing healthcare. A review of published studies, graphs, and data was done to determine the benefit of utilizing home monitoring to treat patients with diabetes.
